Please do the following:

• Fill out the standard BenefitMall census form and the 401(k) and Profit Sharing Plan Client Profile Form Sales Desk and submit.
• Please note any special contribution formula or client wants.
• Please schedule the date and time for the online/conference call presentation (allow for 4 business days for the proposal to reach the client) with you present with the client.
• Standard proposals from the BenefitMall 401(k) and Profit Sharing plan Featured providers are mailed directly to the client within 48 hours (allow for 4 business days for the proposal to reach the client).
• In order to comply with regulatory and legal matters, provider plan specifics and disclaimers must accompany the proposal.
• Special investment providers and third party administrator request will take longer to secure a proposal from. Please allow two weeks to schedule the online/conference call presentation.
